| 1. |
that I, the passenger have come to
see the travel agent on my own free will to request a travel reservation
to local or international destination and upon confirmation and issuance
of the ticket, to pay the said travel agent the cost of the air fare
ticket in ful or in part. |

| 2. |
that I, the passenger have understood the air fare
deposit (which is normally 25% of the total air fare ticket cost)
to be non-refundable upon issuance of the ticket and that said ticket
is neither transferable as explained by the travel agent. |

| 3. |
that once reservation either by phone or in person
has been confirmed by the airline, travel agent will phone passenger
to further confirm the reservation and advise of a ticketing schedule
as demanded by the airline to which passenger may either accept or
refuse. Once refused, reservation is automatically canceled. |

|
| 4. |
that further, upon cancellation of the said ticket,
a $100.00 penalty is due and payable for lost of commissions and processing
of said cancellation. |

| 5. |
that, upon receipt of notice of a confirmed reservation,
travel agent will request passenger to pay the rest of the air fare
cost plus applicable interest and charges from date of ticketing. |

| 6. |
that once ticket has been issued, any changes in
date and time (outbound or inbound) as requested by the passenger
will be subject to a $25.00 fee. |
| |
|
| 7. |
that a further $25.00 fee will be charged for cheque
payment with insufficient funds or if a stop payment was made. |
| |
|
| 8. |
that wolrdwide free baggage allowance is 2 bags
weighing up to 50 pounds or 23 kilograms each. |
| |
|
| 9. |
that anything more than 50 pounds up to 70 pounds,
an excess fee of $60.00 is paid by the passenger for all international
travel and $35.00 for travel within North America. |
| |
|
| 10. |
that there is only one carry-on baggage allotment
consisting of one carry-on bag and one personal item such as handbag,
etc. |
| |
|
| 11. |
that any increase in fuel surcharge will be applicable
for reservations made that is yet to be ticketed. |
| |
|
| 12. |
that we are not responsible for any air miles registration
with Asia Miles, Perks, Mabuhay Miles or any other air miles plan.
However, we will enroll passenger with Aeroplan. |

|
| 13. |
that it is my responsibility to inform travel agent
of my air miles registration number with any of the said above plans
in no. 11. |
| |
|
| 14. |
that travel agent is not responsible for any delays
of the passenger's baggage or lost of it, cancellation of actual date
of departure or arrival by the airline, and hotel accomodations due
to these unfortunate incidents. |
